When your doctors can communicate more easily, you’ll receive better care that meets your unique needs. That’s our goal as the first Accountable Care Organization (ACO) in the St. Louis area. We work together with Medicare to provide high-quality service and care at the right time and in the right setting.

We get that managing your health can be complicated. So, our goal is to try to make things easier – and part of that is providing you with a helpful list of resources that are designed to meet your health needs.

  • Find a class or event to meet your health and wellness needs.

  • Secure online access to your medical record with MyChart.

  • Patients can be referred to BJC Home Care Services by their physicians, case managers, social workers, hospital discharge planners, families, friends or by patient request.

  • For more than three decades, BJC Hospice caregivers have helped patients navigate the end of life’s journey with dignity and compassion. 

  • Imaging through a BJC Hospital offers the latest advancements in imaging technology, which means faster procedures and more detailed images, so your results are right – the first time.

  • OASIS is dedicated to enhancing the quality of life for those 50 and better through programs in the arts, humanities, wellness, technology and volunteer service.

Benefits of the Care Partner Program.  

Managing your health can be complicated. Now you have a Care Partner who works with you and your doctors to help achieve your best possible health. Depending upon your health care needs, your Care Partner may be a registered nurse, care coach or a social worker. Working with a Care Partner is completely voluntary and is a complimentary benefit of being part of the BJC Accountable Care Organization. We encourage you to participate so that we may assist you with managing your health.

Our highly trained, experienced and compassionate team members guide and support you.

A Care Partner assists you with: 
  • Accessing needed health services 

  • Coordinating your care 

  • Managing your symptoms 

  • Maintaining your health and functioning 

Your Care Partner, working with your physician, can: 
  • Coordinate and collaborate on health care services, such as scheduling or rescheduling appointments and medical testing with your doctor 

  • Review prescribed medications and answer your medication questions 

  • Provide individualized education as well as preventative care and self–care tips to help you and/or your family members make more informed decision about your health care options 

  • Help you learn about your health conditions and what you can do to feel better 

  • Provide information on resources, such as transportation services or affordable medication programs 

A Care Partner may benefit patients with the following conditions: 
  • Congestive heart failure (CHF) 

  • Chronic obstructive pulmonary disease (COPD) or asthma 

  • Diabetes 

  • Chronic kidney disease 

  • Ongoing medical problems 

If you go to the hospital, your Care Partner will: 
  • Assess your medical condition and any needs or concerns following your hospital stay 

  • Explain and simplify discharge instructions and other health information 

  • Ensure you have a follow-up appointment with your primary care provider 

  • Provide personalized, one-on-one support and guidance to help you manage your health
