When your doctors can communicate more easily, you’ll receive better care that meets your unique needs. That’s our goal as the first Accountable Care Organization (ACO) in the St. Louis area. We work together with Medicare to provide high-quality service and care at the right time and in the right setting.
We get that managing your health can be complicated. So, our goal is to try to make things easier – and part of that is providing you with a helpful list of resources that are designed to meet your health needs.

Patients can be referred to BJC Home Care Services by their physicians, case managers, social workers, hospital discharge planners, families, friends or by patient request.

Managing your health can be complicated. Now you have a Care Partner who works with you and your doctors to help achieve your best possible health. Depending upon your health care needs, your Care Partner may be a registered nurse, care coach or a social worker. Working with a Care Partner is completely voluntary and is a complimentary benefit of being part of the BJC Accountable Care Organization. We encourage you to participate so that we may assist you with managing your health.

Provide information on resources, such as transportation services or affordable medication programs
